CellTrak is a team of highly focused and passionate entrepreneurs who have spent years understanding the points of pain within the home healthcare and hospice marketplace. Since its founding in 2004 and it’s first client using the service in 2006 CellTrak has delivered cell phone based automation to agencies who no longer want to work with self reported information and paper processes. During this short time period of 5 years, the team has developed the CellTrak name into a brand that is respected, appreciated and has become the industry leader in delivering innovative field staff automation solutions. CellTrak and the CellTraker™ solve major problems for the home healthcare and hospice market. Our forward thinking allows us to continue to penetrate the market and advance our solutions. What is CellTrak?

• CellTrak enables home health paraprofessionals and field staffs of various disciplines to enter patient specific care data into an intuitive CellTraker™ interface. The data collected is sent back to CellTrak’s secure data servers from the point of care, thereby creating a complete electronic visit record that matches the care plan.
• CellTrak’s back-end servers push patient specific care plans and information to field staffs’ CellTraker™ upon arrival at a patient’s home
• GPS monitoring provides accurate tracking of the exact location of homecare staff and recording the actual mileage for each visit.
• Programmed business rules ensure that all visit records match patient care plans and the visit records have been completed before the patient visit can be completed.
• CellTrak’s intuitively easy-to-use web-based administrative system is easily integrated with the software systems residing at the home healthcare agency - thereby completing the end-to-end automation chain.
