ecare

Outcome Based eCare Solution

Enables Care Team Collaboration for the entire circle of care. Solution includes initial patient assessments, patient reassessments and discharge assessments, from the point of care for any home care agency. These assessments are then available to other care givers associated with the patient’s care so that in real-time, any of them can see the results of these assessments and make changes to the service plan provided to the patient. With this technology patient plans of care can be changed just as quickly as they gather the results. Previously these assessments were paper based and were only available to a single care giving individual until the results were turned in, then manually entered into a clinical system.

Solving challenges in Homecare:

CellTrak™ solves the 4 major problems facing home healthcare agencies in Real-Time.

Visit Compliance - 90% of Home Healthcare agencies track employee VISIT COMPLIANCE by the employee subjectively recording their attendance.

Data Collection and error Reporting (EMR) – 90% of Home Healthcare Nurses, Aides and Therapist record their visit attendance, document patient records and visit notes by pen and paper. With an average full time employee producing 25 visits records a week, a mountain of data has to be transcribed and entered into scheduling, billing and payroll systems manually. Resulting in reduced patient care time, increased labor and office expenses, data errors and inaccurate or delayed billing.

Mileage & Time Reimbursements – 90% of Home Healthcare Nurses, Aides and Therapist are expected to honestly report their mileage and time traveled per visit. Resulting in the natural rounding of miles and time traveled per visit and the occasional padding.

Scheduling Communication – 90% of Home Healthcare Agencies communicate schedules to their staff via paper, phone or pager. Resulting in missed or duplicated paperwork, lost patient revenue, and potentially poor service.
