Hospice Palliative Care is in a current state of evolution and with that evolution comes the need to make improvements to clinical documentation. In today’s environment of value, outcomes, efficiency and effectiveness leveraging technology and the use of smart documentation to integrate reliable valid measurement tools that can bring value to the care planning process is particularly important. Determining proactive ways and means to identify trends and opportunities for improvement in care of patients propels an organization forward in achieving best possible outcomes for service. But it is not just reliable and valid measurement tools, integration of best practices and standards into documentation processes supports an organizations’ journey in accreditation, funder compliance and overall excellence. This paper will provide a snapshot to the reader of how the use of smart documentation in an electronic system can strengthen the value proposition of the organization for patients and the larger system while improving documentation, communication and access to patient information.
