Post-Acute care leader automates staff functions and reduces paperwork required to document home care visits

Schaumburg, IL (May 22, 2012) - Chapters Health System, formerly HPC Healthcare, is committed to improving the lives of those affected by advancing age and advancing illness, giving them independence when possible, comfort when necessary and dignity at all times. As one of the first post-acute care systems in the country, Chapters Health offers: Hospice care, Grief services, Senior independence programs and Palliative care across services in Hillsborough, Hardee, Highlands and Polk counties in Florida.

Chapters has been using homecare technologies for clinical and scheduling needs from Suncoast Solutions for several years. They were interested in further automation and reduction of the paperwork required to document home care visits and importantly were interested reducing their mileage reimbursement expenses as gas prices were hitting an all time high. Chapters selected and implemented the new point-of-care visit manager solution from CellTrak™. For the mobile home health and hospice care staff, Chapters selected the robust and secure BlackBerry smart phones running on the Verizon network.

"CellTrak has been very easy to work with and we are pleased with the results in using the mobile solution for our field staff," shared Kathy Fernandez, President and CEO at Chapters, "The integration of CellTrak with Suncoast Solutions is providing value to us in the reduction of paperwork and rekeying of data typically necessary for verifying our visits and paying our staff and importantly provides real-time visibility for our care team to help them maximize the care that they deliver"

Chapters strives to work to educate the patient and family and maintain strict adherence to the physician's orders and plans of treatment, including regular follow-up visits with the physician. Importantly the usage of CellTrak has helped caregivers spend more time on care and less time on paperwork.

Michael K. Wons, President and CEO of CellTrak Technologies added, "Chapters has used the CellTrak solution to help expand the capabilities of the field staff and importantly provide better access to data for the clinical informatics dashboard that helps them visualize care team activities," Wons added "the speed and thoroughness of the implementation team at Chapters was simply amazing and has positioned the solution very effectively to enable the care teams to spend more time on care and less time on paperwork."
Fernandez added, "The robust electronic visit verification technologies from CellTrak will provide additional value to us as we add other healthcare disciplines and field staff to the usage of the solution".

In addition to the integrated visit verification features of the solution, the GPS features available as part of CellTrak takes the guess work out of calculating mileage. GPS tracks the mileage to and from each patient's home and immediately stores it in the system. It also gives staff members the ability to get directions from wherever they are to a patient's home.
